Abstract:
PROBLEM TO BE SOLVED: To provide an exhibition attendant register sheet for reducing the labor of filling for register by attendants and registering securely individual pieces of information of attendants and also provide an attendant register method using the same. SOLUTION: An exhibition attendant register sheet is provided with an inquiry sheet 2 with a filling column for filling answers to items of inquiry and a customer identification code 6 and a label sheet 7 at least with a separable part formed on the inquiry sheet 2 or a sheet 3 formed continuously to the inquiry sheet. The separable section on the label sheet 7 is provided with a customer identification code 14 same as the customer identification code 6 and a pressure-sensitive adhesive layer on which a name card can be so stuck as not to mask the customer identification code 14.
