To prevent a concrete paste from leaking even when vibration compaction is performed by easily following a curved shape of a main body form, and to commonly use a lid form even for a segment with a different curved shape.
The lid form 3 is placed on the main body form 2 provided with a top end face 2a with a curve corresponding to the outer peripheral face of a concrete segment. The lid form 3 comprises a lid main body 4a brought into contact with the top end face 2a and consisting of a flexible body, a rod-like reinforcing member 5 extending in the width direction thereon, and a holding means 4b. The holding means 4b stepwisely connects adjoining joints 6 provided with a pin supporting parts at the center and both ends to variably hold the curve of the lid main body 4a. A hook-like hooking member 7 is hooked on a hooking part 2d provided on the side face of the main body form 2 to give a counter force to the holding means 4b and to press and fix the lid form 3 on the top end face 2a.
