To enable a resident and a company to create application forms without being aware of differences while each local government does not make application formats common and to enable the local government to greatly reduce expenditures since the need to create a homepage for application is eliminated.

An application form input reception processing part 2 of a WEB server 10, when receiving an application for an application form to an arbitrary local government, reads data out of an application form format storage part 3 stored with format information on respective application forms of the local government, displays an application form procedure guide picture on a user terminal 1 which is connected to the server via a network 20 and having made the application, and stores inputted application format data in a storage file 4; and an application form conversion and transmission processing part 5 reads the application form data out of the storage file 4, converts them into data of a computer system of the local government stored in a conversion table 6, and sends the converted data to computers 7 and 8 of the local government.
