Abstract:

To provide a booklet-like form with an envelope which prevents application blanks from being caught in an envelop bottom, and a part of inserted articles from being caught in an envelope.

Advertising paper, application blanks, and envelope paper having an envelope section are layered and pasted to one another into one piece at around one end of a sheet of paper. The envelope paper is formed in a rectangular shape, and rectangular shaped envelope top paper having approximately the same vertical dimension as that of the envelope paper but having a width dimension narrower than that of the envelope paper is layered at the predetermined portion of the envelope paper, and the envelope section is formed by being pasted with an adhesive applied in the shape of a crank at three side-edges of the rear surface of the envelope top paper, and an adhesive layer is prepared on the envelope paper along an opening of the envelope section, and release paper is pasted releasable on the adhesive layer, and a perforation line for cutting is formed on an opposite side on the envelope paper to the opening of the envelope section along a portion where the adhesive layer is prepared, and between the envelope paper and the envelope top paper, an adhesive using a releasable adhesive agent is prepared in parallel with the bottom in the envelop of the envelop section and with a space set up toward the opening from the bottom.
