To provide a business form with sealed letter paper which eliminates the need of pasting when preparing a sealed letter paper to reply and enables personal information to be displayed in an unrecognizable state from outside.
The business form with sealed letter paper form has a rectangular first sheet-like base and a superposed rectangular second sheet-like base. The first sheet-like base is compartmented to a sealed letter paper preparing part and an information base part, having them connected inseparably. The sealed letter paper preparing part includes a sealed letter paper front surface where address information is displayed, and a sealed letter paper rear surface to the address information. The sealed letter paper preparing part can fold from a center part which becomes the border. The second sheet-like base is superposed over a region of the sealed letter paper front surface. A region coated with a pressure-sensitive adhesive is formed in a region compartmented by an outer periphery and cutting perforations of the sealed letter paper front surface, and a region coated with an adhesive is formed to the sealed letter paper front surface except the region coated with the pressure-sensitive adhesive. A releasing agent layer is formed to a surface in contact with the region coated with the pressure-sensitive adhesive of the second sheet-like base.
