To provide a cam follower mounting structure for actualizing reliable mounting of a large cam follower on a mounting base material.

An outer ring is rotatably provided via rolling elements around a bearing surface provided at one end of a stud 2 of the cam follower 1, and a male screw portion is formed at the other end of the stud. The stud 2 is inserted through a cam follower mounting hole 20a of the mounting base material 20, and a washer 40 is mounted between a nut 10 to be threaded to the male screw portion 4 and the mounting base material 20. The washer 40 has a stud through-hole 40a in the center and a plurality of screw holes 40c around the stud through-hole 40a. A set screw 41 is threaded to each of the screw holes 40c to fix the washer 40 to the mounting base material 20. On the nut side bearing surface of the washer 40, rotation preventing pieces 40b, 40b are protruded for abutting on a polygonal outside face 10a of the nut 10.
