To provide a compact disk(CD) holder molding sheet which can provide a storage holder or a mailing envelope on which surface printing and address printing can be done, and which is easy to assemble and inexpensive.

The CD holder molding sheet 2 comprises three-layered printing paper comprising a base layer 15 made of paper or a resin sheet on which printing can be done on a surface, an adhesive layer 16 formed on a rear of the base layer 15, a release layer 17 made of paper or a resin sheet glued to the adhesive layer 16. An opening outline 4 is formed by cut lines 3 extending through all of the layers, and joints 21 and 22 surrounded by separation lines 23 and 24 extending only in the release layer 17. The cut lines 3 are formed in contrast with the outline 4. After the opening outline 4 is separated along the cut lines 3 from the entire sheet, the outline 4 is folded. The joints 21 and 22 are oppositely glued to each other by the adhesion of the adhesive layer 17 exposed on the joints 21 and 22 with the layer 17 removed along the separation lines 23 and 24 to mold the CD holder.
