This composition contains 1-20wt.% based on the dried composition, compound of the formula (R1 is a 1-4C alkyl, a 3-5C cycloalkyl, a 2-6C alkenyl, etc.; R2 is CF3, OCF3, OH, a halogen, etc.; R3 is a 1-4C alkyl, a halogen, a substituted-O-phenyl, etc.; X is CH or N) or its stereo isomer or its salt, and a lipoprotein, preferably a human-derived high density lipoprotein. The lipoprotein content, especially HDL content in a preparation is 0.1-100ml daily. As the compound of the formula, N-(4-trifluoromethylphenyl)-2-cyano-3- hydroxycrotonamide is cited. The composition is useful for the treatment of an acute immunological disease, an acute, a chronic or an ultra acute rejection reaction after an organ transplantation, an autoimmune disease, a type-II diabetes, a colitis, a hepatic fibrosis, a lung cancer, a leukemia, a sarcoma, an ovarian cancer, a rectocolic cancer, a brain tumor, etc.
