The concealing type sheet includes a base material 10a which is divided into an information entry section 30a and a concealing section 40a, an adhesive agent layer provided to one face of the concealing section 40a of the base material 10a and perforations 50a at the boundary between the information entry section 30a and the concealing section 40a. The concealing section 40a is divided into a first concealing section and a second concealing section, wherein the second concealing section is folded at the boundary with the first concealing section and releasably adheres to the first concealing section via the adhesive agent layer.
