To readily and excellently separate a card by fixing a lining structural body of a transparent sheet layer and adhesive paper in a predetermined area at the rear surface side of a continuous form with a unit forms made therein, and forming card sectional areas by a die cut form the surface side in the surface area of the lining structural body.

On the rear side 1 of a continuous form 1 after a desired printing process, transparent sheet layers are glued via an adhesive on the lining structural body 2 in each unit form 6, and a sticking paper is stuck via a second adhesive on the rear side thereof. In order to divide card forming areas 8, formed are die cuts 17 having smaller dimensions than that of the length and width of a lining a structural body 2 that reaches the surface of the adhesive paper from the surface side 1a of the continuous form 1. The card forming area 8 and residual area can be processed by printing. In this structured, the card forming area 8 can be separated between the transparent sheet layer and the second adhesive layer, so the card body can be separated from the unit form through the agency of a separative operation.
