This form is made of an integrally molded crosslinked polymer where a matathesis polymerization catalyst-based catalyst component-made monomer solution A (solution A) is mixed with a monomer solution B (solution B) comprising metathesis polymer ring-like olefin containing the metathesis polymer catalyst-based activator component, thereby polymerizing and crosslinking reacting the raw material mixed solution in a mold so that integrated crosslinked polymer may be available. Side surface portions, which interconnect the forms on the side surface areas, are provided around the surrounding area. A plurality of holes through which a connection clips is inserted are provided within 45mm from the concrete contact surface.
