PURPOSE: To simplify the input and recording for the data on the positions of treated teeth or on the contents of treatment, by providing a signal transmitter which contains a pattern of teeth and a contact through which the position of each tooth can be transmitted on the back side of the teeth pattern.

CONSTITUTION: A transmitting circuit has a panel 2, and the panel 2 displays an adult tooth train pattern 3 and an infant tooth train pattern 4 which are subject to the odontological rules. A contact 9 is provided on the back side of each tooth of the patterns 3 and 4. When the contact 9 is closed, the produced signals are recorded to a recorder 11 via a control circuit 10 in the form of the information. At the same time, the information on the type and contents of the treatment, etc. are fed with ten keys 13, alphabet keys 14 and a group key 15.
