To provide a delivery cut form having a heat-sensitive copy structure without impregnating a surface side with an adhesive provided on a rear surface of an attached cut form of a lower part and without adhering the form of the lower part to a duplicate cut form of an upper side.

The delivery cut form 21 is obtained by superposing a duplicate cut form 23 having a delivery information fill-in portion on an upper side of an attached cut form 22 having a delivery information copy fill-in portion, linearly binding both the forms 22, 23 with a paste 26 and integrating them. A heat-sensitive copy structure 32 for copy filling in the copy fill-in portion of the form 22 by filling in the information fill-in portion of the form 23 is provided, and an adhesive 30 and a release sheet 31 covering the adhesive 30 are provided on a rear surface of the form 22 through adhesive watertight layers 29, 33 and 34.
