To provide a device and a method for forming punched holes in a sheet member for printing which can form punched holes easily in correct positions.

On a base stage 2 and a board, a plurality of positioning holes 4 and 14 are formed, respectively. In each positioning hole, positioning pins 5 and 15 are fitted. An image is printed by an image setter, and at the same time, a press plate in which an image setter punch hole is formed is attached to a punch hole forming device. The image setter punch hole is aligned with the positioning hole 4 or 14 and the positioning pin 5 or 15 is fitted therein. A handle 7 is pressed down and a punch hole is formed by a first punch 11 and a second punch 12. Thus, the punch hole can be formed in a correct position of the image on the press plate.
