PURPOSE: To use a document storing body by sticking it to a back side, or a cover which opens and closes of a note or a file, etc., or its suitable layout sheet.
CONSTITUTION: A small width first continuously connected sheet 5, a small width second continuously connected sheet 6, and a folded sheet 7 are continuously connected to a lower edge of the base sheet 1 via a first lower folding line 2, a first lower center folding line 3, and a second lower folding line 4. Further, small width second right and left continuously connected sheets 11, 11 of the same lateral width as that of the second continuously connected sheet 6, small width second right and left continuously connected sheets 12, 12 of the same lateral width as that of the small width first continuously connected sheet 5, and sticking sheets 13, 13 are continuously connected to right and left edges of the folded sheet 7 via first right and left folded lines 8, 8, first right and left central folded lines 9, 9, and second right and left folded lines 10, 10. Further, the folding sheet is folded on a front side of the base sheet via the first lower center folded line. Besides sticking sheets 13, 13 are folded on a rear side of the base sheet 1 via the first right and left center folded lines 9, 9, stuck onto the rear side of the base sheet to form a flat bag having an inserting bag part equipped with an inserting mouth part on a front side of the base sheet 1, and a sticking paste layer is formed on a rear surface of the flat bag body.
