The entry to a form utilizing an electronic pen is executed by the electronic pen equipped with an ink pen unit or a reading unit for optically reading a dot pattern. The form has a dot pattern region and an entry frame provided in the dot pattern region. By providing a delete point or a check box near the entry frame, a user can indicate the deletion of entries in the entry frame. Further, by the check box, the sequencing of entries extending over a plurality of forms and the distinction information of a form between a new form and a correcting form can be inputted in the check box. Thus, in the forming system of the form utilizing the electronic pen, the user can easily indicate or request the deletion, correction or the like of the entry information through entry actions on the form utilizing the electronic pen.
