To provide a form panel for concrete capable of performing temperature control in such a manner that nearly evens out a temperature of a concrete surface by uniformly adjusting the temperature of a whole form panel.

A form panel 12 for concrete comprises: a form frame 15; a molding plate 16 which is installed on one side face of the form frame 15 and molds a concrete surface; and a rear plate 17 which is installed on the other side face of the form frame 15. An inner space S is formed between the forming plate 16 and the rear plate 17. The form frame 15 has: a rectangular outer peripheral frame material 21; and horizontal frame materials 22 by which the inner space S inside the outer peripheral frame material 21 is separated into a plurality of compartments arranged in the vertical direction. A supply port 31, to supply the upper most compartment of the inner space S₁ with a liquid heat medium for temperature adjustment, is installed at an upper section of the form panel 12. A discharge port 32, to discharge the liquid heat medium from an inside of the lower most compartment of the inner space S_n, is installed at a lower section of the form panel 12. The horizontal frame materials 22 have distribution ports 33 to allow the liquid heat medium to be distributed from the upper compartment of the inner space S_i to a lower compartment of the inner space S_i.
