A form panel 1 includes sheath materials 2I and 2II, and a heat insulation material 3 attached to the inside of the sheath materials 2I and 2II. The sheath materials 2I and 2II include a face plate part 21 to constitute a finished surface and a fastening part 23 having a screw part. The heat insulation material 3 includes an overlapping part 32 at one end and an overlapped part 33 at another end. The neighboring sheath materials 2I and 2II can be connected by engaging, fitting, or overlapping each other. The neighboring heat insulation materials 3 and 3 can be connected to each other by overlapping the overlapping part 32 and the overlapped part 33.
