To provide a form processing device with a small installation area capable of sorting using a widthwise shifting mechanism to widthwise shift a form for reading out information stored in a magnetic stripe, which can be manufactured at low cost.
When the form is discharged from a paper feeding/discharging port arranged in the vicinity of an information read-out means reading out the information from the magnetic stripe using a magnetic head after applying a prescribed process to the form with the magnetic stripe stored with the prescribed information, the form is moved in a direction orthogonal to a carrying direction and shifted widthwise to a first guide or a second guide in accordance with a result of the prescribed process by, for example, a widthwise shifting roller 11 rotatable in a normal and reverse direction provided between the first guide 41 provided in approximately parallel with the carrying direction of the form to adjust the magnetic stripe to a position of the magnetic head and the second guide 42 provided in approximately parallel with the carrying direction of the form to face the first guide. The form is received by either of the guides.
