To provide a compact form reader achieving sufficient magnetic field intensity in the vicinity of the reader, and reducing magnetic field intensity at a distance.
The form reader 1 includes: a loop antenna 6 for reading information recorded in an IC chip 5 mounted on a passport 2 by radio communication; an image reading surface 3 for optically reading image information in a reading region formed on the passport 2; and a digital circuit part 7 for processing the read information or image information. The loop antenna 6 has an asymmetrical 8-shape having a large loop 8 and a small loop 9; the image reading surface 3 is arranged in the large loop 8; and the digital circuit part 7 is arranged in the vicinity of the small loop 9 having magnetic field density higher than the large loop 8. The upper surface of the reader 1 comprises a mounting surface 4 for mounting the passport 2 thereon and the image reading surface 3. The area of the mounting surface 4 is smaller than that of the image reading surface 3.
