To provide a form-rolling apparatus capable of arbitrarily adjusting a phase of form-rolling even if a workpiece is supported from both sides with centers.

A phase adjustment guide 64, of which the inner wall face has a shape formed to be adjusted to a shape of the outer peripheral face of a work W, is attached to an operation center sleeve 60, to which an operation center 53 being one of centers supporting the work W, is attached, and the work W is supported with the operation center 53 in such a manner that the work W is fittingly inserted into the inside of the phase adjustment guide 64. Form-rolling of the work W is conducted by rotating a die 32a and the operation center sleeve 60 with a servomotor in such a manner that the die 32a and the work W synchronously rotate.
