CONSTITUTION: A form is prepared in a predetermined form by folding back and then closely bonding a continuously connected first form 3 and form note 4, wherein a first bonding layer 3e provided in a predetermined outer periphery of the first form piece 3 and a second bonding layer 4e of the second form piece 4 provided on the surface to be in closely adhesion with the first form piece 3 are brought into contact with each other, resulting in the close adhesion between the first form piece 3 and the second form piece 4. Thus, it is formed on the second form piece 4 in a manner including one or more second bonding layers 4e to be in adhesion with the first bonding layer and an area having the second bonding layer provided therein. Accordingly, there is provided a separation piece 4B made to be separated from the second form piece 4 at the time of separating the first form piece 3 and second form piece 4.
