Abstract:
PURPOSE: To favorably open adhered face of forms with marks evidencing unsealing left thereon and without depending on the opening manner.
CONSTITUTION: The forms 20 is constituted of pieces 2, 3 of forms. The pieces of forms are provided with a separation part 7 comprising an adhesive layer 7a and cuts 7b, 7c. The folding of the piece 3 of forms from the connection part adheres the piece 3 of forms through the adhesive layer 7a. When the adhesion piece 3 of forms is opened, the adhesive layer 7a and piece 3 of forms, and the adhesive layer 7a and piece of forms are not separated, and an interstice is made between the cuts 7b and 7c to thereby cut the area including the adhesive layer 7a from the pieces of forms, the piece 3 of forms is opened in a state of the adhesive layer 7a and the piece 3 of forms stuck to each other.
