PURPOSE: To simplify the registration of header data when a new patient visits a hospital.

CONSTITUTION: When a patient code, a patient name, and a birthday are inputted on a header data input screen and an insurant's number is further inputted, a process part 11 detects them, retrieves a header data file 12, and reads out all header data records of other persons having the insurant number. Then only a symbol, a charging rate, an insurant name, an insurant address, and a visit date in data included in the read records are displayed in list form on the screen of a display device 18. When one number in the list is selected, the process part 11 quotes and displays data (symbol, charging rate, insurant name, and insurant address) of part of the record having the selected number at respective corresponding places on the header data input screen.
