PURPOSE: To register the ID information in an identification symbol like a bar code by providing a photographed menu sheet where a photographing menu consisting of the selected items is described in an identification symbol and an identification symbol reader means which reads the identification symbol out of the photographing menu sheet.

CONSTITUTION: In a working state a magnetic card 54 containing the patient information is loaded into a slot 50 of a card reader. Then a bar code reader 52 reads a bar code 58 of a photographing menu sheet 56. Thus the photographing information on the photographing areas, etc., and an ID number are inputted. An operating screen necessary for the input operation is shown on a display 42 together with the data on the patients, etc., which are read out of the card 54. Then an operator inputs the information with the reader 52 based on the operating screen shown on the display 42. Thus it is possible to use such an identification symbol like a bar code, etc., to shorten the input time of the ID information, and to improve the working efficiency.
