PROBLEM TO BE SOLVED: To prevent replacement and/or detect pages without impairing aesthetic design, while maintaining originality and evidentiality of note pages.SOLUTION: An image forming apparatus includes: my note creation function designation means for enabling a function to create note pages of my note used by a user; order ID creation means for providing, for each not page, an order ID that is unique and can make order in creating note pages; order ID unreadable conversion means for converting the order IDs into an unreadable form in providing the order IDs to the note pages; and order ID formation means for forming the order IDs converted into an unreadable form on the note pages.
