The input information retention system that uses a digital pen and a form dedicated for a digital pen comprises: a digital pen having at least a writing means, an optical reading means, a storage means, and a transmission means; an input information processing terminal having at least an input means, a transceiving means, a display means, and a storage means for storing the input information; a digital pen system control server; an information collection server having at least a communication means and a storage means for storing the received information for a specific period; and an information description form on which a dot pattern for reading by a digital pen is printed, and at least a field for specifying the destination of the described information is printed.
