To provide a user-friendly information collection device in which button selection is facilitated.
The information collection device comprises: a plurality of selection buttons 6a-6e and 7a-7c which can be pressed by a user; a communication module 10 capable of storing at least identification information on the user and information on the selection button pressed by the user, and wirelessly transmitting the stored information to a host device; and a cancellation button 4 for cancelling the selection of the information on the selection button pressed by the user. The communication module 10 includes an IC chip 12, and a button cell 14 supplying power source voltage to the IC chip. The IC chip 12 includes a storage unit 21 storing information specifying the pressed selection button and information on a time at which the button is pressed in time series, a control unit 22 controlling storage of information to the storage unit, and a radio communication unit 23 performing proximity radio communication with the host device. The control unit 22 causes, when the cancellation button is pressed, the storage unit to store information indicating the cancellation of pressed information on a selection button having been pressed immediately before the cancellation button is pressed.
