An input information 'in' is inputted to a keyboard 41 and the output information S41 is outputted from the keyboard 41. The information S41 is inputted to a transmission/reception protocol 42 and the output information S42a is outputted from the protocol 42. The information 42a is transmitted to a transmission/reception protocol 51 through a LAN 60, and the output information S51b is outputted from the protocol 51. The information 51b is inputted to an originating destination information conversion part 52, and an identifier S52 is outputted from the part 52 to specify a user serving as an originating destination out of the patient information included in the information S51b. The originating destinations are specified every time the patient information is inputted. The identifier S52 is stored in a memory 53.
