When a copy source application form having the same fields as a copy destination application form is selected from an application form list displayed on a screen, image data of the document and position information data about concealed items specified in the document are read out from a storage location (S301, 302). The copy destination application form is next selected, and the data read out in S302 is copied and stored as position information data about concealed items of the selected document (S303, 304). The copied data is then reflected in the copy destination application form to display an application form so as not to display the concealed items on the screen. If the concealed items are to be corrected on the screen, buttons on the screen are operated to set or cancel the concealment in a flow starting with S307.
