PURPOSE: To improve the easiness of maintenance work by providing a supporting table with an output device, and providing a cover attached to the supporting table with a sheet discharging opening.

CONSTITUTION: An enclosure 1 is provided with a display device 2 and an input device 3 on its upper part, and space 4 is formed below these devices, and its front face is open, and it is provided with guide rails 3 at both sides of its bottom part. The supporting table 6 is provided with casters 7 at the four corners of the bottom part so as to move along the guide rail and to be disengaged. The output device 8 is fitted at the upper part of the supporting table 6, and a control part 9 is fitted inside the supporting table 6. Further, the cover 10 to close the open part of the space 4 of the enclosure 1 is fitted to the supporting table 6, and the discharging opening 11 from the output device 8 and a form receiving tray 12 are provided at the upper and the lower parts of the cover respectively, and handles 13 are fitted at both side parts. Thus, the sheet from the output device 8 can be smoothly discharged, and besides, the form can be easily supplemented.
