PURPOSE: To provide an invitation of an event and an exhibition or the like which has a tactful appearance and contrives efficiency of business such as administration of invited guests.

CONSTITUTION: A card 3 is detachably stuck on the surface of a business form 1 having a continuous form. Setting of an address setting column 4 of the business form 1, an address of the card 3 and setting of a bar code 8 are performed simultaneously and continuously. The respective business forms 1 are cut off after setting and the card 3 is so put into an envelope that the address setting column 4 is seen through the envelope and thereafter sent out as an invitation. Thereby the card-bearing invitation having a tactful appearance is obtained. Moreover efficiency of business such as administration of invited guests is contribed by detaching the card, bringing it with the invited guest and reading the bar code. The card is utilized as a name card and the business form is practically used for a questionnaire. Further simultaneous setting is enabled for the business form and the card and missending is prevented by putting both sides into the envelope as they are.
