The patient registers its own dietary information, exercise information, inspection information, etc., in the IC card 3 through the use of the barcode reader 15. At that time, the telephone set 11 calculates even ingestion calorie corresponding to the meal and caloric consumption corresponding to exercise. The registered contents can be inquired by the liquid crystal display equipment 113 of the telephone set 11. The patient brings the IC card 3 to the hospital. A host device 5 on the hospital side reads information inside the IC card 3 with the IC card reader/writer 533 and displays it as a graph in terms of the sequence in a display.
