A donut-plate shaped printed circuit board 11 and an optical fiber positioning plate 21 are arranged in multiple steps. A number of LEDs 15 are mounted on the printed circuit board 11. A number of corresponding optical fibers 26 are positioned and mounted by the optical fiber positioning plate 21. The optical fibers 26, that are connected optically to the LEDs of the printed circuit board 11 at a previous stage one by one, are inserted into insertion parts 12 and 22 of the printed circuit board 11 and the optical fiber positioning plate 21 at the next stage, and at the same time, are bundled with the optical fibers 26 that are connected optically to the LEDs 15 at the next stage and are pulled out.
