PURPOSE: To make maintenance work easy by installing a sheet member supply device which supplies a sheet member to be present between folded cut forms at the upstream side of a transport line for the cut form, and supplying the sheet member to the cut form.

CONSTITUTION: A film 2 is held between a rotary drum 37 and a holding roller 36 and is sent downward. In this case, the film 2 is held by a suction force from a hole 42 connected to a vacuum chamber 45, starting gradually with the preceding end side of the film, and is carried downward. Air is ejected from the hole 42 which has held the film 2 by suction when the hole 42 passes a B point sequentially, because the hole is connected to a high pressure chamber 46. Then the film 2 is peeled off the drum 37. If the preceding end part of the film 2 is positioned at the B point, almost the center 1c of a cut form 1 passes this position. The film 2 is held by suction by a hole 56 provided on a folding roller 51 through the cut form 1. The cut form 1 passes through an area between both folding rollers 51, 52 from the center 1c side, and subsequently, is folded doubly, holding the film 2 in between.
