CONSTITUTION: A paper-discharge port 29 is formed to the top face of a main body housing 22 shaped in an erected wall form flat in the longitudinal direction while a paper-feed port 27 is formed near the lower edge of a front, and an image reader 45 is mounted onto a form carrying path 33.
