Abstract:
PROBLEM TO BE SOLVED: To mitigate the trouble to issue documents by providing a receipt confirmation sheet on a pasteboard for sending in a freely separable manner and collectively outputting delivery information to the address entry part and the address entry part for receipt confirmation of the pasteboard for sending, and separating the receipt confirmation sheet from the pasteboard.
SOLUTION: A receipt chop (signature) part 12 sectioned by a cutting perforation of a sheet is formed at one of the corner areas of an upper position sheet and a non- adhesive receipt confirmation sheet 2 is attached to a lower position sheet in a freely separable manner. In addition, an address sheet 3 is arranged in an area other than the receipt confirmation sheet 2 of the upper position sheet 5 and is laminated on the lower position sheet in such a manner that the sheet 3 is hardly peelable. Further, in an area other than the receipt confirmation sheet 2 and the address sheet 3, a reply postal card piece 4 which is formed by sectioning with a cutting perforation 10 deep enough to reach the lower position sheet from the upper position sheet in a freely separable manner, is provided. Besides, a pasteboard 1 and an article to be delivered are stored in a see-through bag so that the receipt confirmation sheet 2 and the address sheet 3 can be visually checked, and a part scheduled for opening a removing aperture for removing the receipt confirmation sheet 2, is provided.
