CONSTITUTION: An insurance content report 1 has a main slip 3 and a copy slip 4 connected through a cutting sew line 2, wherein print columns 5-11 of the slip 3 and print columns 13 of the slip 4 are so set that printed characters are directed perpendicularly to each other such that, even if a size of the slip 4 is decreased, necessary information amount can be obtained, and a sheet surface can be effectively used to be decreased in size.
