The server device for displaying a request acquisition panel on a display part of equipment being an input terminal comprises: a layout information acquisition part 303 for acquiring layout information on a layout of a screen in which arrangement positions of constituent components are set; a constitution presentation part 305 for presenting information showing the layout and constituent components to a screen creator on the basis of constituent component list information; a constituent component operation acquisition part 307 for acquiring constituent component specification information for specifying the constituent component that is input on the basis of the presentation by the constitution presentation part and acquiring action information on an action associated with the constituent component shown by the constituent component specification information; a constituent component information acquisition part 302 for acquiring the constituent component information on the basis of the constituent component specification information; and a panel edit part 309 for generating a request acquisition panel by using the screen layout information, the action information, and the constituent component information.
