To prevent an error at the time of an order for an additional print by a method wherein an image photographed on a negative film is printed as a positive image on one sheet member and also an additional print check space for entering the information on the additional print of the positive image is provided thereon.

A sheet 1 for an order for an additional print is made of thermosensible paper or the like and shaped like a rectangle and all images photographed on one negative film are printed as positive images 2 on the surface of the sheet along the opposite side edges thereof. Between the positive images 2 arranged in these opposite side edges, a plurality of additional print check spaces 3 extending in the vertical direction and arranged in the lateral direction are provided. The additional print check spaces 3 are divided into first and second regions 5 and 6 by a line 4 and, moreover, the first and second regions 5 and 6 are divided by transverse lines 7 into a plurality of data entry spaces 8 and 9 wherein marks 10 with the blank inside are printed. The first and second regions 5 and 6 are provided for entering the information on the additional print of the positive images 2 arranged on the left and right sides.
