A first sheet-like base is compartmented by folding perforations, and cutting perforations are formed at an equal interval from the folding perforations. In addition, the cutting perforations are formed along the upper edge and the lower edge of the first sheet-like base, and a folding paper part and an intermediate paper part have almost the same width size, and the folding paper part has a width size which is superposed over a region located in the right side of the cutting perforations of the intermediate paper part. In addition, an adhesive layer region is formed in the center, on the upper surface of a second sheet-like base adhesion part, and a pressure-sensitive adhesive layer region is formed in its external region. Besides, a releasing agent layer is formed in a region which is superposed over the pressure-sensitive adhesive layer region on the back of the second sheet-like base. The second sheet-like base has a cut formed between the adhesive layer region and the pressure-sensitive adhesive layer region, and only the part where the releasing agent layer is formed of the second sheet-like base, can be released from the pressure-sensitive adhesive layer by means of the cut.
