To provide a slip for a sealed letter foldable in three which contains private information entered on a piece of paper for preparing a sealed reply letter as such a content is not discernible from outside.
This slip with a sealed letter form is made up of a first sheet-like base of a laterally long rectangular shape, and a second sheet-like base which is of the same size as the first sheet-like base and adhesively superposed on the first sheet-like base. In addition, the slip has a zone outside of a sewing machine of two perforation lines for cutting formed laterally along the upper end side and the lower end side of a first adhesive paper part of the first sheet-like base and a zone where an adhesive layer is formed in a nearby zone defined along the left edge side of the first adhesive paper part. Further, on the back of a second sheet-like base, a release agent layer is formed in the zone superposing a zone where the pressure-sensitive adhesive layer of the first sheet-like base is formed. Besides, on the second sheet-like base, a notch is formed in a location between the zone where the adhesive layer of the first sheet-like base is formed and the zone where the pressure-sensitive adhesive layer is formed.
