Abstract:
PURPOSE: To provide a trocar having a sleeve 12 fixed on the abdominal wall 78 and also provide an airtight seal 76 to a piercing incised region so as to keep the open state of the abdominal cavity, obtain optimum vision and facilitate the operation of an instrument. CONSTITUTION: After a trocar 10 and the tip of a sleeve 12 are inserted in the abdominal cavity, the trocar 10 is drawn out of the sleeve 12, and expanded by an injector through a balloon 40 expansion pipeline. The sleeve 12 has a ratchet 60 on the outer surface, and a bumper 44 has ratchet 60 teeth on its inner surface. Accordingly, it freely slides in the directions of the bumper 44 and the balloon 40 and the abdominal wall 78 is clamped between the bumper 44 and the balloon 40. The bumper 44 is kept from being moved backward by the ratchet 60 teeth, and a tube for inserting a freely manual operated surgical instrument in the abdomen is formed. A means for releasing the bumper 44 and contracting the balloon 40 is also provided to remove the sleeve 12 from the abdomen.
