To provide a template with a case which is shifted crosswise from the case and receives a continuous entry of numerals without their lengthwise disorderly arrangement.

This template with a case consists of a template 1 having notched parts 3 corresponding to numerals and symbols for tracing the numerals and symbols for entry, a case 11 deposited on a desk in such a manner that the case 11 is stacked on the template 1, an opening part 12 for exposing the notched parts 3 provided in the case 1, a sliding part 13 which is provided on the upper and lower end parts of the case 11 and supports the template 1 in slidable manner, and referential part 14 which is provided on the sliding part 13 and transfers the template 1 in parallel, coming into contact with the template 1.
