CONSTITUTION: A delivery confirmation slip 9 on which an entry column of an adressee data, the distribution day of a catalogue 2 or the name of a distributor is printed is provided at a part of a sheet 4 on which the addressee data of a customer and the data column of the customer are printed so as to be capable of being cut off. A cutoff line 23 for forming the opening part for taking out the delivery confirmation slip 9 is provided at the position of a home delivery envelope 3 to which the delivery confirmation slip 9 corresponds when the sheet 4 equipped with the delivery confirmation slip 9 is housed in the home delivery envelope 3. The taken out and collected delivery confirmation slip 9 are arranged in order so as to be easily delivered by a home delivery driver and returned to a mail-order firm after distribution to become a delivery completion report. The mail-order firm performs the advance degree of home delivery or the confirmation of home delivery based on this stub to control a customer.
