The tooth flank form rolling apparatus 1 for performing the form rolling of a tooth flank on an outer circumferential surface of a shaft-like member 70 comprises a pair of form rolling dies 3 slidable in the directions opposite to each other while the shaft-like member 70 being held, a rotational supporting pin 4 for rotatably supporting the shaft-like member 70, and a rotation-stop pin 5 for stop the rotation of the shaft-like member 70. Each form rolling die 3 including an approach forming tooth flank part 31 and a finish forming tooth flank part 32 forms a member holding unit 33 for holding the shaft-like member 70 before starting the forming by the approach forming tooth flank part 31.
