To organize a system for issuing circulation questionnaires for reducing loads on the issuer of the questionnaire and allowing quickly obtaining responses.

A confirmation requester registers items to be confirmed as circulation, in a response control database 2 of a Web server 1 via a Web terminal 4 and issues the circulation to respondents desiring it. The Web server 1 comprises a circulation questionnaire issuing section 14 for receiving the items of circulation questionnaire issued via the Web terminal of the requester and register each item added with a unique identifier in the database 2 on the Web for transmitting the items to the Web terminals of the desired respondents, and a circulation questionnaire response collecting/transferring section 15 for receiving the responses via the Web terminals of the respondents for registering them in the database 2 on the Web and tabulates the responses for each identifier for transferring them to the Web terminal of the requester.
