The present invention relates to an automatic pill inspection apparatus comprising: a pill feeder (10) for supplying pills (p); a pill supply device (100), which is for the automatic pill inspection apparatus and aligns and transfers the pills (p) supplied from the pill feeder (10) one by one; and a pill alignment and transfer device (200), which is for the automatic pill inspection apparatus and determines sequential exposures of one side and the other side of the transferred pill (p) and transfers the determined pill, wherein the automatic pill inspection apparatus further comprises: an upper side detection camera (300) provided at one side of an upper drum (210) so as to inspect an upper side of the pill (p); and a post-inversion detection camera (310) provided at one side of a lower drum (240) so as to inspect a lower side and each side of the pill (p), wherein the pills (p) determined to be defective products, according to the inspection results of the upper side detection camera (300) and the post-inversion detection camera (310), are selectively discharged to a first supply pill discharge box (330) by the compressed air, and the normal pills (p) are discharged to a normal pill discharge box (350).
