Abstract:
It is representing the Dynamic Anti-rotative Plate for additional osteosynthesis. The goal of invention is to improve the Kuntscher's surgery method. The construction is based on the scientific findings that cyclic-axial compression stimulates the osteogeny and remodeling of the bone and that the movements of rotations and distraction are slowing the healing time. The technical problem in treatment of the bone fractures by Kuntscher's nail is insufficient possibility to stabilize the rotations' and distractions' movements of the bone fragments. When you use the Dynamic Anti-rotative Plate with the Kuntscher's nail, one half of the plate (1) shall be fixed to one part of the bone's fragment and the other part of the plate (2) with its opening (6) shall be leaned on the other bone's fragment. If the plate is placed properly the most optimal harmony of three important mechanical factors for the neatly healing of the bone fracture shall be achieved. The cyclic-axial compression is possible and at the same time the movements of rotations and distractions are impossible at the place of the fracture. The result of it, is that the healing time is shorter and the healing with rotational deformation of the extremity as result is impossible. Using of this plate together with Kuntscher's nail can be successful in the healing of the bone fractures of humerus too.

1. Dynamic Antirotative Plate for additional osteosynthesis characterized by openings for fixed and dynamic parts.

Description:
DYNAMIC ANTI-ROTATIVE PLATE TECHNICAL FIELD Dynamic Anti-rotative Plate is used in medicine for the additional osteosynthesis during the treatment of bone fractures by Kuntscher's nail.

The goal of this invention is to improve Kuntscher's surgery method.

TECHNICAL PROBLEM Insufficient possibility of stabilization of rotations'and distractions'movements of bone fragments is erasing as technical problem in the treatment of bone fractures by Kuntscher's nail.

BACKGROUND ART The invention is based on following scientific knowledge : 1) Cyclic-axial compression stimulates osteogeny and remodeling of the bone 2) Movements of rotations and distractions in the area of the fracture are slowing down the healing or healing may end as rotational deformation of the extremities.

The osteosynthesis with all of those mechanical factors in optimal harmony, will be the most acceptable one. That osteosynthesis will be the one making possible the cyclic-axial compression of bone fragments and making impossible bone fragments' distraction and rotation at the same time.

Küntscher has applied the nail in the treatment of long tubular bones'fractures in year 1939. He has achieved the cyclic-axial compression of bone fragments but he has not achieved the necessary stabilization of rotations'and distractions'movements.

Kuntscher and the other authors tried to solve this problem.

Küntscher tried to achieve the elastic fixing by extension of the bone marrow canal in year 1952.

Herzog introduced de-rotation wires for shinbone in year 1953.

Kaessman modified the nail and he tried to achieve measurable compression in year 1966.

In year 1968. Küntscher has added his method, fixing the nail with screw only for the upper or lower bone fragment and that resulted with dynamic fixation. When he fixed nail on both of bone fragments that resulted with static fixation.

The analysis of previous solutions results with the conclusion that neither one of solutions can completely provide the optimal mechanical conditions for healing of the bone fractures. That means providing of the cyclic-axial compression and at the same time making impossible rotations'and distractions'movements of the bone fragments at the place of fracture.

When it is about the plates for osteosynthesis of bones diaphysis, it is necessary to say that all of them are differently named according to the shape, function or authors.

Purpose of all of the plates is to achieve a rigid osteosynthesis and the plates have nothing similar with"Dynamic Anti-rotative Plate" DISCLOSURE OF INVENTION The primer goal of the invention is to provide optimal mechanical conditions for the healing of bone fracture during the treatment of the fracture with Kuntscher's nail.

That means to make cyclic-axial compression possible and to make rotations'and distractions'movements of the bone fragments impossible at the place of fracture.

Further goal of the invention is to reduce the healing time for bone fractures and to prevent the healing of the fracture in rotational deformation on technically simple and practicable way.

The final goal of the invention is to improve Küntscher's surgery method.

The usage of the Dynamic Anti-rotative Plate for additional osteosynthesis, as the part of the treatment of bone fracture with Kuntscher's nail, makes cyclic-axial compression of bone fragments possible on simple technical way. At the same time, its completely makes rotations'and distractions'movements of the bone fragments impossible at the place of the fracture.

All this is possible because of the new invention's construction solution characterized by fixed and dynamic parts.

BRIEF DESCRIPTION DRAWING The sketch of"Dynamic Anti-rotative Plate"is on Fig. 1.

The longitudinal base of the plate is flat and the transferal base is gently bent down-balloon shape, with concavity on the side that leans on the bone.

At the half of the plate's body (1) are individual round openings (3), (4), and (5).

Their profile is conical plunged openings.

At the half of the plate's body (2) there is one longitudinal oblong opening (6) with vertical and straight walls (7).

BEST MODE FOR CARRYING OUT THE INVENTION I will describe the details of this invention illustrating it on the sketch attached.

The plate itself is short and narrow plate with individual openings (3), (4), and (5) on the one half of its body (1) used to connect the plate with the bone with the screws. On the other half of the plate (2) there is one longitudinal opening (6) with vertical and straight walls. The opening (6) is of wider diameter than diameter of screw's heads.

When it is decided that the bone fracture, stabilized by surgery performed by Kuntscher's Open Method, is not providing enough stability of rotations'and distractions' movements of bone fragments, the additional osteosynthesis shall be performed with Dynamic Anti-rotative Plate.

The screws used shall be settled in only one cortical.

The plate shall be placed on the most suitable place during the surgery. The one half of the plate (1) with individual openings for the screws (3), (4), and (5) shall be placed on one of the bone fragments and firmly fixed by screws. That is the fixed part of the plate. The other half of the plate (2) with longitudinal, oblong opening (6) shall be leaned on the other bone fragment. At least two screws with or without pads shall be placed through longitudinal, oblong opening (6), starting with the furthest place from the fracture's fissure with the condition that the part of the opening (6) the closest to the fracture's fissure has to be available. Since the opening (6) is with wider profile than screws'heads placed through it, the half of the plate (2) is not strongly connected with the bone. That is the dynamic part of the plate.

The half of the plate (2) with oblong opening (6) and the screws placed to the bone through it, shall be used as the conductor for directing of the compression forces to the fracture's fissure. At the same time it shall make the movements of rotations and distractions of the bone fragments, impossible at the place of the fracture.

The plate shall be removed by surgery after the finished procedure.

The goals of this invention are achieved by the new characteristic construction's solutions.

The Dynamic Anti-rotative Plate shall be made from the standard alloy materials, so called medical steel, in different dimensions.

MODE APPLICATION The invention brings significant improves comparing to the previously offered solutions for the problem of rotations'and distractions'stability of the bone fragments during the treatment of the bone fracture by Kuntscher's nail. It is technically simple to use it with usual instruments. That makes the usage practical and economical. There is no exposure to X-rays.

The bone fracture stabilized in surgery by Kuntscher's nail and Dynamic Anti- rotative Plate makes the time of fracture's healing shorter, with absolute guaranty that the healing will not end with rotational deformation. This confirms economical and medical justifiably of usage of the plate. Beside the usage in healing of the bone fractures on lower extremities, Kuntscher's nail can be used with the Dynamic Anti- rotative Plate in the healing of the bone fractures of humerus.
