Abstract:
The Fixation System comprises rods (1 ), hing connectors (4), plates (3), screw holders (11 ) and Hemi-arches (7). Said Hemi-arches contain one hole in one end edge in oblique manner and vertical plan is guiding a screw (12) inward in a fixed angle toward the center of the Hemi-Arch (7) while at the other end we can apply screw fixation device (10) threw a hole in the other end which guide and fix another screw (10) to cross the first one at the center of bone (at center threaded part of the screw or nearby) in order to obtain a stable fixation of bone fragments in a crisscross manner and then leave a sufficient space of soft tissue and bone for suitable operation for lengthening and corrections, since hand have relatively small bone, without injury of tendous and neurovascular structures, because of the screws crossing the bone obliquely from side where no nerves or vessels are passing.

Claims:

Claims to be protected

1- The Hemi Arches as drown in (Fig.N.2; Item number 2, part number SF5, and Fig.N.2 a).

2- The unit of Hemi-Arches with screws in crisscross fixation manner. Obtaining stable fixation and avoiding injury to the tendons and neurovascular structures (Assembly 2; Fig.N.6, 7 and N.8).

3- The ability of modifying the system to apply to the Hemi- Arch a screw holder and screw fixation device which with a screw inserted to the bone out ward , to obtain more stability;(Assembly 3;Fig.N9,10,l land N.12).

4- The ability of modifying the system to combine of units of (Assembly2 and Assembly 3) to allow suitable fixation and lengthening and treatment of various cases.

5- The modified Hinge system; First Type (Salamehfix 5 a) as drown in (Fig. N. 1, 2, 3, 4, and N.5).

6- The modified Hinge system; Second Type; (Salamehfix 5 b) as drowns in (Fig. N. 13, 14, 15, 16,17and N.18).

7- The modified Hinge system; Third Type (Salamehfix 5 C) as drowns in (Fig. N.19, N20, N.21and N.22)

8-The ability of modifying the system to add hinges connecting between Hemi-Arches with rods in order to correct axial deviations and complex deformity ;(Female Hinge Connector ;Item 4,Part N.SF4 in Type Two; Salamehfix 5 b, and Item 3 in Third Type; Salamehfix 5 C), and ( Male Hinge Connector ;Item 6,Part N.SF3 in Type Salamehfix 5 b).

9-Usisng various Rods depending of the case to treat as (Slotted Treaded rod 1; Fig.N.2 and Fig.N.2b) in First Type (Salamehfix 5 a) and Threaded rod 2 Item 1, Part N.SF1 / SF 2, and Slotted Threaded Rod 3; Item 2, Part N.SF2) in Second Type (Salamehfix 5 b), the two last ones have at its end a miner in diameter end for connection of hinges and nuts, which secure connection and enable rotation and correction of axial deviations.

10- Possibility of adding plates (Connecting Plate l;Item3,Part N.SF6 as in second type salamehfix 5 b) which allows assembling the system in two parallel rods to support the system when we need more strength and stability in correcting contractures and treatment fractures and deformities.

11- The principle of crisscross fixation manner on which this system acts in treatment of acquired and congenital deformities of hand, fingers and metacarpal bones.

12 - The possibility of using this system in treatment all kinds of Fractures of hand and metacarpal bones in hand and foot, there deformity and Lengthening.

Description:

The external fixation hinge system for treatment of hand and metacarpal Fractures and lengthening (Salamehfix 5).

Technical Status:

The congenital and acquired deformities of Hand, fingers and metacarpal bones is a relatively complex problem in terms of small fingers and the lack of space through which screws and skewers could be entered in order to lengthen and correction of axial deviations of these fingers and metacarpal bones. Thus we are facing a real problem since that the device that should be used must be relatively small with specific technical specifications such as the ability of stable fixation in a way that enables us to make an osteotomy close to the metaphysial aria in order to accelerate the production of callus which - in turn- accelerates the process of healing and lengthening of the fingers and the metacarpal bones to be treated.

Since small fingers and metacarpal bones make it relatively difficult to apply a device that has such technical specifications, I have invented a system that used in lengthening and correction of axial deviations of fingers and metacarpal bones of the hands and feet.

The principle on which this device acts is to fix screws in crisscross

manner inside the bone in a way that leaves sufficient space to perform

the surgery in the right place close to the

metaphysial area. In addition, this device is featured by its stable fixation of bone fragments.

Moreover, we may add to this device hinged parts that enable us to lengthen and correction of axial deviations of bones in all directions.

This system can be modified for treatment of joint contractures, osteo arthritis, dislocation or subluxatons of metacarpal-phalanges or inter phalanges joints by adding special hinges.

Technical description:

The external fixation hinge system for treatment of hand, metacarpal Fractures and lengthening (Salamehfix 5). The hinge system is modified in three types;

First type ;(Salamehfix 5 a) (Fig. N.I, N.2, N3. N.4, N.5). Consists of relatively two small Hemi-arches ( Item number 2 ,part number SF5,and as drown in Fig. N. 2 a )which contain one hole from one end side threw which inserted a screw (Item number 4, Part number SF9 ) in an oblique manner to the direction of center of the hemi-arch to crisscross with other screw (Item number 4, Part number SF9 ) which inter threw a screw fixation device (Item number 3, part number SF7 ) .this screw fixation device can applied and fixed threw a hole on the other end of the Hemi-Arch ,this screw fixation device can rotate to allow the screw to crisscross the other first screw /without touching him or light touching / in the center of the bone or nearby it will be in the middle or nearby the center of the threaded part of each screw ,

this unit of details I named as ( Assembly 2 ) which drown in ( Fig. N. 6, N.7.N.8) the details of this unit are drown and named with Part List in (Fig. N.8) , so we can choose the angle of insertion of the second screw so we can crisscross the bone in center this fact allows stable fixation of bone fragments, this two screws of the Unit of (Assembly 2) can inserted in one end of the bone and at the same time leave a sufficient space between these two screws and the other two screws inserted threw the other (Second) Hemi-Arch /another or second Unit of (Assembly 2) /to the other end of the bone , for operation which we need mostly at the metaphyses /nearby end of bone /for example to lengthen the bone ,its known the best place for lengthening , and at the other idea leave more soft tissue for lengthening with miner scaring, or even its better in cases of fixation of fractures ,simples or comminuted of the hand , fingers and metacarpal bone since the hand and fingers and metacarpal bone have relatively small long for fixation, .The screws will inter in the vertical plan of the bone leaving enough space for fixation and lengthening. The Hemi-arches are connected by means of threaded rods (Item 1 Part number SF8). Washers (Item 5, Item 6) and Hex nuts (Item 9, Item 10) are used to secure connection between parts of the system. Hexagon Socket Set Screws (Item 7, Item 8) are used to secure the screws. Distraction or compression of bone fragments we can do by rotating of the nuts clockwise of on other direction.

The second type of this system is (SALAMEHFIX 5 b); (Fig.N.13, N.14, N.15, Nl 6.N.17, and N.18). Consists of relatively small two Hemi- arches (Item 7, part number SF5) as drown in details ( Fig.N.15a) which contain one hole from one end side threw which inserted a screw (Item 12, Part number SF9) in an oblique manner to the direction of center of the hemi-arch to be inserted perpendicular to the bone ,and on the other end edge are applied a screw holder (Item 11, Part number SFlO) which contain a hole threw which

applied a screw fixation device (Item 10, part number SF7). Threw screw fixation device inserted screw (Item 12, Part number SF9) which we can apply in deferent angels and directions according to the condition. So we can rotate the screw fixation device to insert the screw outward or inward in any needed angel to the bone and then cross the bone in oblique manner, this type of application make more distance for fixation and then more stability, this complex of details I collected as a Unit, I named (Assembly 3) are drown in (Fig. 9, N.10, N.I 1, and N.12) and the details with Part list are drown in (Fig.N12). The Hemi-arches are connected by means hinges are attached between them according to the condition to be treated using Male hinge connector (Item 6, part number SF3) are applied threw the hole in center of the First Hemi-arc (first Assembly 3) and fixed to the Hemi arch by means of nuts and washers, this male hinge connector can fixed to Female hinge connector (Item 4, Part number SF4) by means of Hex-Head Bolt (Item 9), by slight releasing this Bolt we can allow rotation of hinges on each other, the female Hinge Connector are attached and fixed to the slotted threaded rod (Item 1 Part number SFl / SF 8) or slotted threaded rods (Item 2, Part number SF2) and by means of nut /Hex Nut, Item 5/, which secure connection by releasing this nut we can allow rotation and displacement of bone fragments, this threaded rod inter threw the center of the Second Hemi arch (Second Unit of Assembly 3)and fixed by means of nuts which allows fixation or distraction or compression between Hemi Arches by rotating the nuts clockwise or on other direction . a connecting plats (item3, Part number SF6) are used to support the hinge system in two parallel rods threaded rods (Item 1 Part number SFl / SF8) or slotted threaded rods (Item 2, Part number SF2),so we can add one connecting plate to the First Hemi-Arch (First Assembly),which inter threw one hole in one end of the plate threw the end

of the Male Hinge connector and secured to the Hemi-Arch by Nut in a vertical plan of it, and the second connecting plate attached to the second Hemi-Arch threw one hole in one end of the plate which inter in the Slotted threaded rod and secured to the Hemi-arch in a vertical plane , threw the second hole in the second end of the first plate inter a rod which attached to a female hinge connector, this one secured with Bolt to another female Hinge connector which attached to a threaded rod and secured with a nut , the rod is entered threw the second hole from the second plate on the second Hemi-Arch and secured with a nuts , by rotating these nuts clockwise or another direction we can produce distraction or compression of bone fragments, this fact add more stability in cases when we need to correct angular deviations or reduce of dislocations or subluxations of hand joints or even in foot small joints and also in treatment of joint contractures of the hand, Washers (Item 13, Item 14) and Hex nuts (Item 5, Item 15) are used to secure connection between parts of the system.

Hexagon Socket Set Screws (Item 16, Item 17) are used to secure the screws. In more simple cases the system can be modified to use the Unit of (Assembly 2) as in the First Type instead of (assembly 3), or combine the unit of (assembly 2) in the one compartment of the system and the Unit of (Assembly 3) in the second compartment of the system. The Third Type of this system is (SALAMEHFÏŠX 5 C); this type is modified for treatment of cases which need lengthening from one side and compression of bone fragments on the another side ,the System are drown in (Fig.N.19,N.20,N.21,and N.22) this system contain of three unites of ( Assembly 2) as in First type (Salamehfix 5 a) as in (Fig.N6,N.7and N.8) and as in (Fig.22,Item 5) the Part List as in(Fig.N.8) and the details of this unit are explained previously in the First Type, The first Hemi-arch are connected by means of slotted threaded rod (Item 1 Part number SFl / SF8)

or slotted threaded rods (Item 2, Part number SF2) as in Second Type (Salamehfix 5 b) are inserted threw the hole in center of the First Hemi- arc in ( Assembly 2) and fixed to the Hemi arch by means of nuts and washers, on the other side of the threaded slotted rod are inserted the second (Assembly 2) threw the hole in the middle of the Hemi-Arch and secured with Hex Nuts (Fig.22, Item 4) and hinges are attached between them according to the condition to be treated using Male hinge connector (Item 6, part number SF3) are applied threw the hole in center of the First Hemi-arc (first Assembly 3) and fixed to the Hemi arch by means of nuts and washers, this male hinge connector can fixed to Female hinge connector (Item 4, Part number SF4) by means of Hex-Head Bolt (Item 9), by slight releasing this Bolt we can allow rotation of hinges on each other, the female Hinge Connector are attached and fixed to the slotted threaded rod (Item 1 Part number SFl / SF8) or slotted threaded rods (Item 2, Part number SF2) and by means of nut /Hex Nut, Item 5/, which secure connection by releasing this nut we can allow rotation and displacement of bone fragments, this threaded rod inter threw the center of the Second Hemi arch (Second Unit of Assembly 3)and fixed by means of nuts which allows fixation or distraction or compression between Hemi Arches by rotating the nuts clockwise or on other direction done to treat more complicated cases ;In Cases when we need correction of deformity we can modify this type of system by adding hinges like in second Type /Salamehfix 5b / Male hinge connector (Item 6, part number SF3), and Female hinge connector (Item 4, Part number SF4), and slotted threaded rods (Item 2, Part number SF2) are connected between the Hemi-Arches and we can rotate during lengthening or even during fixation, we can rotate the plan of this hinges depending of plan of deformity or even make displacement of bone fragments if needed by connecting of the hinges from one edge side to other edge side of the other hinge part .

The system can be modified In cases of longer bone or cases when we need more stability depending on the case we treat we can connect another Hemi-arch for lengthening the bone from one side and compression from the other side or use a screw holder threw which inserted the screw fixation device (Item number 3, part number SF7 ) threw which inserted a screw(Item number 4, Part number SF9 ) so we can rotate the screw fixation device to insert the screw outward to the bone to make more distance for fixation and then more stability.
